Application for Occupational Therapy Department’s Graduating Student Awards, to be announced and presented at May graduation. Scholarships to be awarded are the Andy Thompson Award, and the Barbara Umlauf Award.

  4. The Andy Thompson Award was established to recognize a deserving student with an interest in serving individuals with Traumatic Brain Injuries (TBI) or psychiatric disabilities. Applicants interested in the Andy Thompson Award: please submit a one page, typed statement addressing the following: Describe your personal reasons for interest in Traumatic Brain Injuries (TBI) or psychiatric disabilities. Please include experiences volunteering or working with individuals who have a TBI/psychiatric disability, and future plans to work in this area (such as requesting fieldwork in this area). Class assignments focusing on TBI/psychiatric conditions are also relevant.
  5. The Barbara Reber Umlauf Award is awarded each year to graduating students within the Occupational Therapy Deparment with competitive GPAs that are members of the Student Occupational Therapy Association (SOTA). Applicants must display significant volunteer and work experiences relating to their MOT degree.
    • Volunteer Experience: please upload a list of your volunteer experiences, including non-paid work and pre-professional activities other than SOTA in which you have participated in the past two years. Experience for which you received class credit should not be included. Please include the semester/year in which the activity took place, a brief description, and an estimate for number of hours served for each.
    • Work Experience part 1: please upload a list of your previous PAID work experiences from the first year of the program through this current semester. Please include the semester/year in which you held each job, whether you were full-time or part-time in the MOT program, an estimate for number of hours worked per week, as well as your employer's name and phone number for each.
    • Work Experience part 2: provide supporting documentation to show you held each job listed in the previous section. Appropriate forms of documentation include: a copy of your W-2, statement from supervisor on employer letterhead stating hours worked in each semester, or any other documentation proving your employment during that time.
